Srinagar (Kashmir): Insomnia or sleeplessness is a sleep disorder which is defined as repeated acute or chronic inability to initiate or maintain sleep, despite the adequate time and opportunity for sleep. Insomnia is usually caused by psychological stress, physical illness, psychiatric disorder such as depression and anxiety, post traumatic stress or panic disorder and medications like beta blockers, decongestants, stimulants, theophyline etc. Use of tobacco, caffeinated products and illegal drugs may also cause Insomnia.
Women are at more risk for Insomnias than men. It is usually attributed to the hormonal imbalance in females. A report from a general consensus revealed approximately 33% of adult population report one or more symptoms of Insomnia.
In allopathic system of medicine the recommended treatment is cognitive behavioral therapy(CBT) and sedative hypnotics and sedating depressants are commonly prescribed drugs. In case of CBT the problem of patient compliance occurs. The patients do not follow the recommended advice properly hence the results are poor. These drugs have side effects like they lead to cognitive slowing, ataxia, forgetfulness, nausea, weight gain and in some patients they may aggravate Insomnia.
Unani physicians have been successfully treating Insomnia through regimenal therapy since ancient times. Regimenal therapy is a special technique/physical method of treatment in Unani system of medicine. Nutool (irrigation) is one of these regimenal therapies specifically used for the treatment of Insomnia. Nutool refers to pouring or dripping of drugs on specific body parts.
An observational single case study was done at Regional Research Institute of Unani Medicine, Kashmir, University Srinagar, on a female patient of age 20 diagnosed with chronic Insomnia for a period of 15 days. Roghan-e-Babuna (Matricaria Chamomile oil) was used for irrigation therapy and duration of Nutool was 30 minutes.
The 15 days study was divided into four visits with an intervwal of 5 days and on each visit progression or regression of symptoms were noted. The Mijaz(Temprament) was assessed on the basis of Ajnasse Ashra proforma designed for the assessment of Mijaz.The assessment of efficacy was based on subjective(headache and wakefullness) parameters and objective(Insomnia severity index).Headache was rated with scores from 0 to 4 on VAS scale. Objective parameters were assessed on the basis of Insomnia screening questionaire(ISQ index).
It was found that VAS scores for wakefulness are reduced from 4 to 1 from baseline to third follow up. Insomnia severity questionnaire showed significant (60%) improvement in objective parameters.
From the above study it can be concluded that the Nutool produced significant effect in Insomnia and hence can be recommended for management of Insomnia.
